Participants build new relationships and learn new skills to help them manage all the things needed to improve their health, as well as to help keep them active in their lives and relationships. | |

Otago Exercise ProgramThe Otago exercise program for falls prevention is designed for those who may be frail and still living in the community. Otago is taught by Physical Therapists and Physical Therapist Assistants primarily in the home setting. The curriculum is delivered over 8 weeks with the participant doing the exercises independently between sessions. The Otago Exercise Program requires a scheduled assessment and session planning through a local provider. Otago is covered under Medicare and Medicaid. Walk with EaseAll 18 sessions are led by an in-person, Arthritis Foundation-certified leader. The Group program has three sessions per week for six weeks. Each workshop is 1hr of group sharing, education, and participants walk at their own pace for 10 to 35 minutes. All participants receive the Walk With Ease guidebook with information on how to get ready to walk, begin walking and stay motivated to continue walking. Walk With Ease is a also a great addition to other prevention and self-management programs. | |
Walk With Ease Distance LearningWalk With Ease Distance Learning combines self-paced walks with information about health-related topics delivered via Zoom or phone meetings.
It is a six week workshop that allows participants to work on their own at their own pace to increase their physical activity.
Participants receive a workbook.

Walk With Ease Self DirectedAn individual uses the Walk With Ease Guidebook to complete the six-week program on his/her own. Communication materials are designed to be distributed weekly to program participants through a weekly engagement series. Participants completing the Self-Directed are encouraged to walk at least three times per week. All participants receive the Walk With Ease guidebook with information on how to get ready to walk, begin walking and stay motivated to continue walking. Walk With Ease is a also a great addition to other prevention and self-management programs. | |
YMCA Blood Pressure Self-Monitoring Distance LearningThe YMCA Blood Pressure Self-Monitoring (Y-BPSM) program is a 4-month program designed to help adults with hypertension lower and manage their blood pressure. It focuses on home self-monitoring of one’s blood pressure using proper measuring techniques and individualized support and nutrition education for better blood pressure management. Topics and skills covered include better blood pressure management, increased awareness of triggers that elevate blood pressure, and enhanced knowledge to develop healthier eating habits. Class includes two 10-minute personal consultations per month regarding blood pressure management, 1-hour monthly nutrition education seminars, and self-measurement of blood pressure at least 2 times per month. Participants must be at least 18 years old, diagnosed with high blood pressure, must not have experienced a recent cardiac event, must not have atrial fibrillation or other arrhythmias and must not be at risk for lymphedema. At this time, the class is only available in English.

Instructor Training- A Matter of BalanceThis one day Leader/Coach training prepares the participant to deliver A Matter of Balance: Managing Concerns About Falls. The program requires 2 Leaders/Coaches to lead the community class. Participants in the training will practice following the leader manual content and process; recognize that for older adults, the fear of falling is a significant risk factor in falling; use effective modeling strategies in leading the group exercises; and recognize how to handle people and situations appropriately. Class includes the Training Manual.

Instructor Training- Tai Chi for ArthritisInstructor training for Tai Chi for Arthritis a falls prevention exercise program is two days focusing on the basic six core movements and the advanced six extension movements. Instruction will cover the essential Tai Chi principles, precautions, warm up and Qigong (exercise to improve relaxation and vital energy). Successful participants in this training will receive a two year certification to teach the class. The certification is through Dr. Paul Lam and Tai Chi for Health. The class instructor is Master Trained by Dr. Paul Lam. This training includes an instructional DVD and manual. | |

Leader Cross Training - CDSMP/Tomando Control de su SaludDesigned for the Spanish speaking community, Tomando Control de su Salud is the Spanish version of the Chronic Disease Self-Management Program. Cross training is two, eight hour days, taught entirely in Spanish. The program was developed and initially researched by Stanford University. A co-leader is required to deliver these classes in the community. This training, like the class, is highly participatory, where participants engage in various group discussion topics. Leader Training - OtagoThe Otago exercise program for falls prevention is designed for those who may be frail and still living in the community. Otago is taught by Physical Therapists and Physical Therapist Assistants primarily in the home setting. The curriculum is delivered over 8 weeks with the participant doing the exercises independently between sessions. The Otago Exercise Program requires a scheduled assessment and session planning through a local provider. Otago is covered under Medicare and Medicaid.
