Visit the CDC information page for readiness and recognition. Two-day DPP Coach trainings are taught by a Master trainer. You may apply for Coach training after your organization has applied for DPP recognition from the CDC.
Contact the Living Well Center of Excellence to learn how to become a provider of any of the Stanford Chronic Disease Self-Management Education Programs, including Living Well (for all chronic conditions), Living Well with Diabetes, Cancer: Thriving and Surviving, Living Well with Pain, Tomando Control de su Salud, or Programa de Manejo Personal de la Diabetes.
Diabetes Self-Management Education can be certified by the American Diabetes Association or accredited by the
